Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to effectively address the unique needs of each patient.

• Facilitate open communication among family members, guardians, and other stakeholders to monitor and report on individual progress toward treatment goals.

• Develop and provide supervisors with a detailed schedule for appointments, home visits, and assessments to enhance workflow efficiency.

• Implement and coordinate home visits to ensure accurate observations and gather relevant data, contributing to holistic patient understanding.

• Ensure that program plans are executed effectively and in alignment with clinical best practices.
\n
• Actively participate in Interdisciplinary Team Meetings and other relevant seminars to promote collaborative care strategies.

• Prepare meticulous monthly and quarterly progress reports to evaluate treatment efficacy and adjust care plans accordingly.

• Organize training sessions to ensure that all team members are proficient in the implementation of the treatment programs and documentation protocols.

• Conduct follow-up interviews and home visits to provide ongoing support and reassess client needs, ensuring a continuity of care.

• Maintain comprehensive records, ensuring that all documentation is available and accessible to authorized parties in a timely manner.

• Serve as a liaison with the public and the larger mental health community to promote a supportive network and achieve developmental goals.

• Assist in creating and refining training materials aimed at enhancing our service delivery and program structures.

Qualifications\n
