Home Health Registered Nurse - SOC/ROC/Recertification

Alliance Home Health Care & Nursing Services is a family-owned, Medicare- and Medicaid-certified home health agency based in Plymouth, Minnesota. Since 2007 we have helped Twin Cities residents recover and stay safely at home. Our team provides skilled nursing, physical, occupational and speech therapy, private duty nursing and in-home support services. The role We are hiring a full-time, salaried Registered Nurse to own Start of Care (SOC), Resumption of Care (ROC) and recertification visits across our West Metro territory. You will manage your own field schedule, routing and visit sequencing within required OASIS timeframes, and report to the Director of Nursing. Pay: $90,000 - $104,000 per year, based on experience Schedule: Full-time, field-based (West Metro, Minnesota) Responsibilities Complete SOC, ROC and recertification assessments within required timeframes, including accurate OASIS data collection

• Perform comprehensive assessments: functional, cognitive, medication, fall/safety, wound/skin and pain
• Verify and document homebound status, skilled need and medical necessity
• Develop and update individualized plans of care with physicians, patients, families and the care team
• Identify changes in condition, elevate urgent findings to the physician and Director of Nursing, and perform skilled nursing interventions as ordered
• Educate patients and families on disease management, medications and home safety
• Coordinate with therapists, social workers and our intake and scheduling teams
• Keep accurate, timely documentation in KanTime EMR, in line with CMS, Minnesota and agency requirements

Required qualifications - Previous home health or home care experience

• Current BLS/CPR certification
• Valid driver's license, acceptable driving record, current auto insurance and reliable transportation for West Metro travel
• Cleared Minnesota DHS background study before patient contact
• TB screening per MDH/CDC requirements before patient contact

Preferred qualifications SOC/ROC/recertification experience (strongly preferred)

• OASIS experience or COS-C certification
• KanTime or comparable home health EMR experience
• Familiarity with Minnesota home care regulations and PDGM
• ICD-10 coding experience in home health
• Experience with complex chronic conditions (CHF, COPD, diabetes, wound care, IV therapy)

Physical requirements Frequent driving between homes; lifting up to 50 lbs occasionally and 25 lbs frequently; frequent standing, bending and kneeling; possible exposure to communicable disease and varied home environments.
