Pediatric Outpatient Staff Pharmacist PRN

Organization Vanderbilt University Medical Center (VUMC), Nashville, Tennessee. Job Summary Provides leadership in optimal product preparation and dosing of medications, and provision of patient specific drug information to include patient specific dosing. Aides in the prevention and reporting of medication errors and adverse drug reactions according to policy. Supports quality relationships with health care professionals, families, and patients; and the pharmacist is actively involved in being further educated and providing education in all appropriate ways. Position Highlights Retail environment filling discharge prescriptions, completing refills for complex patient populations, setting up mail, and helping families of patients directly. Clinical component includes evaluating doses and indications with their MAR, submitting prior authorizations, and making interventions with providers directly. Flexible shift schedule: at least 1 day/week or 2 days/every other week once fully trained and efficient, between the hours of 6:30am - 11:00pm. Weekend rotation every 6 weeks, 3:00pm - 11:00pm. Key Responsibilities Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost‑effective use of medications. Provides outstanding patient care by optimizing medication preparation and resolving medication‑related problems. Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy. Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients. Additional duties may be assigned. Technical Capabilities Pharmaceutical Quality Assurance & Compliance (Advanced): Specialized knowledge of rules and regulations, process improvement, quality assurance techniques, compliance with federal, state and local regulations and VUMC policies. Pharmaceutical Computer Literacy (Novice): Proficient understanding of computers and use of pharmaceutical computer applications supporting moderate‑level patient care issues. Pharmaceutical Drug Preparation & Dispensing (Novice): Proficiency in reviewing, preparing and dispensing prescribed medications; general principles of aseptic technique for sterile preparations. Medication Management (Intermediate): Proficiency in medication management for complex practice; determines clinical appropriateness, dosage, contraindications, adverse effects, interactions, monitors lab results, and provides drug information. Mentoring & Coaching (Novice): Provides constructive training and feedback, removes roadblocks, and shares experiences to aid learning. Position Qualifications Certification: Licensed Pharmacy Technician in Tennessee. Experience: Less than 1 year relevant work experience. Education: Bachelor's degree (required).
