  Position Summary

The Intake Coordinator manages all Community Supports referrals and new clients except PAIR Team referrals. This position serves as the primary intake coordinator, answers the intake phone, answers the door, distributes mail, completes eligibility determinations, coordinates enrollments, and ensures timely processing of referrals. The Intake Coordinator serves as a primary point of contact for individuals seeking services and support. This position is responsible for conducting client screenings and intakes, coordinating referrals, connecting clients to community resources, and ensuring timely follow-up throughout the service process. The Intake Coordinator plays a vital role in helping clients navigate housing, employment, healthcare, and other support systems while maintaining accurate records and communication.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

• Answer the Community Supports intake phone and respond to referral inquiries.
• Receive and process all Community Supports referrals except PAIR Team referrals.
• Complete outreach, eligibility screenings, and intake assessments.
• Verify Medi-Cal eligibility and obtain authorizations.
• Schedule eligible clients with appropriate programs and case managers.
• Maintain documentation in InSync and agency systems.
• Communicate intake status with referral sources.
• Coordinate with internal staff and community partners.
• Answer the door.
• Distribute mail.
• Maintain HIPAA compliance.
• Perform other duties as assigned to support program operations.

Requirements:

  Minimum Qualifications (Knowledge, Skills, and Abilities)

• Experience in case management, care coordination, or human services preferred, but not required.
• Strong organizational, communication, and documentation skills.
• Experience with electronic case management systems preferred.
• Bilingual Spanish preferred but not required.

Work Environment · This position requires working in an office setting. · Interaction with clients experiencing housing instability and other challenges is frequent. 

Working Conditions None: The worker is not substantially exposed to adverse environmental conditions (as in typical office or administrative work). 
Physical Requirements Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
