Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Conshohocken, Pennsylvania. The role is full‑time, 40 hours per week, with an 8‑hour shift and ongoing employment type. Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager. Support patients in Norristown, PA and surrounding areas. Deliver quality patient care, maintaining clinical competence and meeting critical charting deadlines. Qualifications RN license in the state where the work is performed. Graduate from an approved school of practical nursing. One year of RN experience. Current driver’s license and ability to drive approximately 20% of your day to patient locations. Ability to self‑manage time and schedule. Demonstrated experience with tablets, mobile phones and EMR software. Successful completion of a background check, including a drug screen for medical and recreational marijuana.
