Key Responsibilities

• Conduct comprehensive evaluations of patients in the home care setting, assessing their physical abilities, mobility, and therapeutic needs.
• Develop and implement individualized treatment plans based on thorough assessments, utilizing a wide array of therapeutic modalities such as manual therapy, exercise prescriptions, and mobility training.
• Regularly track and document patient progress, making adjustments to treatment plans as necessary to ensure optimal recovery and goal attainment.
• Educate patients and their families about treatment methods, potential outcomes, and the management of conditions using assistive devices when required.
• Collaborate with interdisciplinary teams, including occupational therapists and speech therapists, to provide comprehensive care for patients.
• Engage in continuous professional development, participating in workshops, reviewing relevant literature, and attending professional society meetings to refine your skills.

Patient Population / Client Focus

Your patient population will encompass a variety of individuals, ranging from those recovering from orthopedic injuries to patients rehabilitating after surgeries or managing chronic conditions. This role requires a high level of adaptability, as you will tailor your treatment methods to accommodate diverse needs and goals, striving to enhance mobility and alleviate pain.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• Current and valid State License in Physical Therapy is mandatory.
• Strong verbal and written communication skills for effective interaction with patients, families, and team members.
• Basic proficiency in word processing and computer skills for documentation and communication purposes.
• A flexible and adaptive approach to patient care, with a strong commitment to improving patient outcomes.

Preferred Experience

While a minimum of 2 years of experience as a Physical Therapist is preferred, we welcome recent graduates who are eager to learn and grow within our supportive environment. Prior experience in rehabilitation or home care settings will be considered an asset.

Compensation and Benefits

The salary range for this position is competitive, ranging from $100,000 to $120,000 annually, commensurate with experience.
