Description

• Conduct evaluations for patients in inpatient or outpatient settings and formulate tailored treatment plans
• Implement physical therapy techniques emphasizing mobility, strength, balance, and functionality
• Work collaboratively alongside nurses, physicians, case managers, and other rehab professionals to enhance patient outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercise regimens
• Document evaluations, treatments, and progress in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary conferences to assure coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
