Key Responsibilities\n

• Conduct thorough assessments to evaluate clients' needs, strengths, and challenges, allowing for personalized care strategies.

• Develop, implement, and manage comprehensive support plans tailored to individual clients' situations.

• Provide crisis intervention, enabling clients to navigate immediate emotional distress effectively.

• Maintain detailed documentation of client interactions, therapeutic processes, and progress toward goals to ensure high-quality service delivery.

• Actively participate in case conferences, offering evidence-based insights and collaborating with other healthcare professionals to enhance the overall treatment approach.

• Connect clients with community resources and external agencies to facilitate their recovery journeys.

• Serve as a key worker, coordinating care across various disciplines and ensuring consistent, supportive follow-up.

• Monitor and evaluate the effectiveness of treatment plans, making necessary adjustments based on client feedback and progress.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals affected by significant life challenges, including those experiencing grief, trauma, and life-limiting illnesses. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n

The annual salary for this position ranges from $75,000 to $90,000, commensurate with experience and qualifications.
