Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional, social, and financial needs of clients, ensuring a tailored approach to care.

• Facilitate individual and group counseling sessions addressing issues such as trauma, adjustment disorders, grief, and coping strategies.

• Coordinate with community organizations to arrange necessary post-discharge services, promoting continuity of care and resource accessibility.

• Provide timely crisis intervention and emotional support to clients and their families, ensuring they have the tools they need during difficult times.

• Develop and implement effective discharge plans that connect patients with appropriate community resources and follow-up care.

• Collaborate with interdisciplinary teams to monitor client progress, enhance treatment efficacy, and improve overall patient satisfaction.

• Maintain accurate and comprehensive documentation in compliance with legal and organizational standards.

The annual salary for this position ranges from USD 70,000 to 100,000, commensurate with experience and expertise.
