Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of patients.

• Work collaboratively with interdisciplinary medical teams to develop comprehensive care plans that address both mental and physical health concerns.

• Provide individual and group counseling sessions focusing on issues such as grief, adjustment to life changes, and coping strategies.

• Facilitate connections to community resources, ensuring patients receive essential post-discharge services for continued care.

• Educate clients and their families on treatment options and available resources, empowering them to make informed decisions.

• Document patient interactions and care processes accurately, adhering to confidentiality laws and organizational policies.

Qualifications\n
