Key Responsibilities\n

• Utilize your educational background, training, and clinical expertise to conduct initial assessments and formulate independent psychosocial diagnoses for patients.

• Collaborate with patients to create personalized treatment plans focused on their unique challenges and aspirations, ensuring a patient-centered approach.

• Deliver behavioral interventions based on well-established evidence-based practices, including solution-focused therapy, behavioral activation, and motivational interviewing, tailored to each patient’s needs.

• Coordinate care with a multidisciplinary team, including psychologists and primary care providers, to develop a holistic treatment approach that encompasses all aspects of a patient’s health.

• Monitor patient progress through systematic tracking of treatment responses, both in person and virtually, ensuring that goals are met through continuous evaluation and adjustment of treatment plans.

• Assist with psychotropic medication management by providing adherence support, monitoring side effects, and evaluating effectiveness as directed by collaborating medical providers.

Qualifications\n

• A Master's degree in Social Work from an accredited program.
