This full-time position is primarily remote, enabling you to work comfortably from your home while providing essential mental health services to patients at a Hospice Center in Corpus Christi, Texas. Our flexible work schedule accommodates the needs of our clients, including potential evening and weekend hours to provide them with the timely support they require.

Key Responsibilities\n

• Utilize your extensive education and training to independently assess and formulate psychosocial diagnoses for patients, ensuring tailored care plans that address their specific needs.

• Collaborate closely with patients to create individualized, patient-centered treatment plans that focus on their emotional and psychological well-being.

• Implement a variety of evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and behavioral activation to enhance therapeutic outcomes and assist patients in achieving their goals.

• Work in tandem with other healthcare providers, including clinicians, psychologists, and primary care physicians, ensuring an integrated approach to each patient's care.

• Monitor patient progress through systematic tracking of treatment responses and feedback, utilizing both in-person and virtual communication to maintain an ongoing assessment of their mental health journey.

• Support and contribute to psychotropic medication management as advised by medical providers, focusing on treatment adherence, side effects, and overall effectiveness in collaboration with the patient's healthcare team.

This role focuses on providing mental health support to patients receiving hospice care, a delicate and vital area of practice. You will engage with individuals experiencing significant life transitions, helping them navigate the psychological complexities associated with end-of-life care. Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

• You must hold a current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in Texas.

• At least one year of post-graduate clinical experience as an independently licensed therapist is essential.

Compensation and Benefits\n
