Key Responsibilities\n

• Utilize your clinical training to conduct comprehensive psychosocial assessments, developing individualized treatment plans in partnership with patients and their families.

• Implement evidence-based therapeutic interventions including solution-focused therapy, motivational interviewing, and behavioral activation tailored to the unique challenges faced by hospice patients.

• Maintain ongoing communication with a multidisciplinary team including medical providers, psychologists, and other specialists, ensuring a coordinated approach to holistic patient care.

• Monitor and evaluate treatment responses systematically, adjusting care plans as necessary in collaboration with patients, to track progress toward established goals.

In this role, you will work primarily with patients receiving hospice care, encompassing a diverse array of individuals facing terminal illnesses. This population requires sensitivity and specific knowledge regarding palliative care, grief, and loss. Qualifications\n

• A Master's Degree in Social Work from an accredited program is required.

• Hold a current, valid, and unrestricted LCSW license in the state of Texas.

Compensation and Benefits\n
