Key Responsibilities\n

• Utilize your expert knowledge to independently formulate psychosocial diagnoses and assessments tailored to each patient’s unique situation.

• Collaboratively develop, implement, and continuously review patient-centered treatment plans aimed at addressing both medical and emotional needs.

• Employ evidence-based behavioral interventions, including solution-focused treatment and motivational interviewing, to facilitate positive patient outcomes.

• Coordinate care with an interdisciplinary team of clinicians, including psychologists and primary care providers, ensuring a seamless experience for patients.

• Regularly monitor and document patient progress through systematic tracking of treatment responses, using both in-person and virtual tools.

• Support psychotropic medication management, partnering with medical providers to ensure adherence while evaluating effectiveness and potential side effects.

Patient Population / Client Focus\n

This role primarily focuses on patients in hospice care, who may be coping with terminal illnesses and require extensive emotional and psychological support. You will engage with patients and families during a critical time, providing not only therapy but also essential resources and guidance, ultimately impacting their quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) within the state of Oregon.

• At least 1 year of experience practicing as an independently licensed therapist, ideally within a mental health or hospice setting.

Preferred Experience\n

Experience in hospice or palliative care settings is highly desirable, as is familiarity with interdisciplinary collaborative work. Additionally, proficiency in various therapeutic modalities and a history of implementing evidence-based practices will enhance your candidacy.

Compensation and Benefits\n

The annual salary for this position ranges between USD 80,000 and 100,000, reflecting the qualifications and experience of the chosen candidate. Additional compensation includes an annual incentive bonus, comprehensive health benefit plans, a 401(K) with employer matching contributions, and reimbursement for license renewal fees.
