Key Responsibilities\n

• Ensure the smooth transition of patients by confirming discharge appointments with both the patients and their loved ones prior to discharge.

• Communicate effectively with referral sources and outpatient providers, facilitating continuity of care at key points: admission, midpoint, and discharge.
\n
• Facilitate patient transfers to more intensive levels of care when needed, ensuring that all necessary steps are taken to maintain patient safety and well-being.

• Lead psychoeducational and process-oriented therapy groups, fostering a supportive environment for patients to share and learn from one another.
\n
• Manage and de-escalate crises within therapy sessions and the milieu, employing established crisis intervention techniques.
\n
• Participate actively in clinical outreach and program tours, representing Horizon Mental Care Services and contributing to community awareness about mental health resources.

• Document patient interactions and treatment plans accurately and in a timely manner to comply with best practices and regulatory standards.

Patient Population / Client Focus\n

You will work primarily with individuals undergoing detoxification from substances, providing them with critical psychosocial support. This population often presents with complex needs related not only to substance use but also to co-occurring mental health conditions. Your role will involve assessing their emotional and social needs and connecting them with appropriate resources to aid their recovery journey.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Valid and active Licensed Clinical Social Worker (LCSW) designation in the state of California.

• Demonstrated ability to communicate effectively in both written and verbal forms.

• Expertise in establishing and maintaining healthy boundaries within patient relationships.

• Capacity to produce accurate documentation of services in accordance with industry standards.

Compensation and Benefits\n
