Key Responsibilities\n

• Conduct thorough and culturally sensitive biopsychosocial assessments, addressing mental health, substance abuse, and medical needs to provide individualized treatment.

• Deliver both individual and group counseling sessions, facilitating discussions that support emotional processing and coping strategies specific to the challenges faced by our patients, particularly in the hospice context.
\n
• Create and maintain meticulous electronic documentation of mental health evaluations, treatment plans, progress notes, and crisis intervention outcomes in compliance with organizational standards.
\n
• Utilize crisis intervention techniques effectively, supporting patients during periods of emotional distress and escalating care requirements to medical professionals as necessary.
\n
• Act as a liaison between patients and community resources, providing referrals to essential services such as medication assistance and support groups.
\n
• Collaborate with Quality Improvement teams to design and implement effective data collection processes to enhance individual and organizational performance metrics.

Patient Population / Client Focus\n

This role primarily serves adults and families within the hospice care environment, often facing complex emotional and psychological challenges due to terminal illnesses. Your ability to connect empathetically and provide expert guidance will be pivotal in improving their overall care experience and outcomes.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
\n
• Current and valid License as a Clinical Social Worker (LCSW) in the state of California.

• A minimum of three (3) years of post-licensure experience in a clinical or social work environment, preferably in hospice or palliative care settings.

• Demonstrated expertise in counseling techniques, crisis intervention methodologies, and the ability to make independent clinical judgments.

• Proven skills in group facilitation, treatment planning, and community resource navigation.

Compensation and Benefits\n

The salary range for this position is competitively set at USD 115,000–130,000 per year, commensurate with experience and qualifications. In addition, we offer a comprehensive benefits package that includes:
