- # Physical Therapist (Outpatient) Wexford Health + Wellness PavilionApply: On-site: Wexford PA, 15090, 12311 Perry Highway: Full time: Posted Today: J288387## **Company :**Allegheny Health Network## **Job Description :****Schedule**4 days/wk between M-F with flex day off, no weekendsRotational hours/days: (2) 12 hour days and (2) 8 hour daysAll shifts have daylight start**GENERAL OVERVIEW:**Evaluates and treats patients with injuries or diseases resulting in loss of physical or functional abilities; supervises work performed by support personnel; effectively communicates and collaborates with patients, families and members of the healthcare team. **ESSENTIAL RESPONSIBILITIES:*** Performs initial and on-going assessments of patient's condition and establishes goals and plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.* Performs therapy interventions utilizing standard, and, possibly specialized, physical therapy techniques and skills as appropriate for the age and condition of the patient.* Completes documentation to comply with department, hospital and regulatory standards.* Participates in departmental initiatives for operational improvement, including committees and educational offerings.* Performs other duties as assigned or required. **QUALIFICATIONS:**Minimum* Bachelor’s Degree* State licensure as a Physical Therapist or eligible.Preferred* Master’s Degree**Additional Employment Requirements:*** CPR – American Heart Association* Act 34 Criminal Background Clearance Certificate* Act 33 Child Abuse Clearance Certificate* Act 73 FBI Fingerprinting Criminal Background Clearance Certificate* Current state driver’s license.***Disclaimer:*** *The job description has been designed to indicate the general nature and essential duties and responsibilities of work performed by employees within this job title.
