Key Responsibilities\n

• Serve as a primary liaison with clients, families, and outpatient providers to facilitate seamless communication and care continuity.

• Engage with insurance companies for pre-authorization and utilization management to ensure that client treatment plans are supported and funded appropriately.

• Participate in discharge planning and aftercare services, developing strategies that promote holistic well-being and reduce readmission rates.

• Provide therapeutic meal support, modeling a positive and healthy relationship with food, to clients focusing on nutritional guidance and emotional support.

• Craft individualized care plans that reflect the unique circumstances and intersecting identities of each client, ensuring that services are sensitive and tailored to their personal journeys.

• Conduct assessments and evaluations, utilizing strong diagnostic skills to inform treatment decisions and optimize client outcomes.

Patient Population / Client Focus\n

You will primarily be working with a diverse clientele that includes individuals from various backgrounds and age groups, with a particular focus on older adults. Your role is crucial in addressing mental health challenges that arise in different life stages, particularly during transitions that may impact their emotional and psychological well-being. Compensation and Benefits\n
