This full-time, remote position based in Wichita, Kansas, focuses on enhancing the lives of seniors by providing critical social services and support. Work Environment\n

This role is designed for individuals seeking a flexible work environment that encourages the use of technology and supports remote engagement. You will maintain regular communication with clients and their families through phone calls, video conferencing, and electronic health record (EHR) systems. Although the majority of your interactions will be conducted remotely, occasional in-person visits may be necessary based on client needs. Additionally, while the standard working hours are Monday to Friday, from 8:00 AM to 4:30 PM, your schedule will need to be flexible to accommodate client visits and any urgent situations that may arise.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, considering their emotional, mental, and social well-being.

• Collaborate closely with clients and their families to develop and implement individualized care plans tailored to their unique goals and circumstances.

• Provide counseling and emotional support, assisting clients in effectively managing life changes such as health issues, loss, and transitions.

• Advocate for clients’ rights and access to essential community resources, ensuring they receive the support they require.

• Work in conjunction with a multidisciplinary team, which may include healthcare professionals, caregivers, and other social workers, to coordinate holistic care strategies.

• Regularly monitor clients’ well-being through visits, assess living conditions, and address any safety or care concerns.

• Facilitate discussions and conflict resolution among family members to ensure collaborative support for seniors.

• Educate clients and their families about available community resources, programs, and support groups to empower social engagement and improve life quality.

• Maintain accurate documentation of client interactions, assessments, care plans, and progress notes, while adhering to ethical standards and policies.

• Stay updated on best practices, regulations, and trends in social work, ensuring compliance with the relevant state and professional standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
