Work Environment

This role is based onsite at our state-of-the-art Rehabilitation Center in Sunnyvale. Key Responsibilities

• Conduct thorough assessments of resident capabilities and challenges to inform the development of personalized treatment strategies.
• Design tailored therapy programs focused on enhancing strength, mobility, and functional independence, ensuring that residents' unique needs are met.
• Utilize various therapeutic modalities, including manual therapy techniques and exercise regimens, to address specific physical impairments and promote recovery.
• Regularly monitor and document residents' progress, making necessary adjustments to treatment plans to optimize efficacy and support rehabilitation goals.
• Collaborate with an interdisciplinary team to create cohesive care plans, participating actively in case discussions and contributing to comprehensive resident care.
• Educate residents and their families on effective home exercise strategies and wellness practices to extend rehabilitation benefits beyond clinical settings.
• Ensure adherence to all applicable state and federal regulations, as well as maintain the highest professional standards of practice.

Patient Population / Client Focus

The residents at our Rehabilitation Center primarily consist of elderly individuals, often dealing with recovery from surgeries, degenerative conditions, or traumatic injuries. Qualifications

• Active licensure as a Physical Therapist in the state of California.
• Bachelor's Degree in Physical Therapy from an accredited institution; Master's or Doctorate is preferred.

• Demonstrated organizational skills to manage several cases simultaneously while maintaining quality care.

Preferred Experience

• Prior experience working in a skilled nursing facility or rehabilitation setting, particularly with geriatric populations, is highly desirable.
• Familiarity with electronic health record (EHR) systems and clinical documentation protocols is a plus.
• Experience in developing and implementing outcome-based therapy programs that enhance patient engagement and recovery success.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
