Key Responsibilities\n

• Conduct in-depth psychosocial assessments through structured interviews, direct observations, and reviews of patients' medical histories to determine emotional, social, and environmental factors affecting their recovery.

• Provide compassionate counseling and support to patients and their families, addressing adjustment difficulties and enhancing emotional well-being during the rehabilitation journey.

• Collaborate closely with a multidisciplinary rehabilitation team to formulate tailored, goal-oriented treatment plans that consider the unique psychosocial needs of each patient.

• Assist patients in navigating and accessing essential community resources, including vocational rehabilitation programs, support groups, and housing assistance services.
\n
• Educate patients and families about available community resources and post-rehabilitation services that can facilitate ongoing support after discharge.
\n
• Participate in comprehensive discharge planning, ensuring seamless transitions for patients returning home or moving to alternative care settings.

Patient Population / Client Focus\n

Our patient population includes individuals recovering from a variety of conditions such as substance use disorders, neurological challenges, orthopedic surgeries, and other medical issues requiring rehabilitation. As part of our team, you will help patients enhance their coping strategies, navigate recovery hurdles, and achieve greater autonomy in their daily lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.

Compensation and Benefits\n
