Key Responsibilities\n

• Collaborate with an interdisciplinary rehabilitation team to design and implement individualized treatment plans, incorporating patients' psychosocial needs and treatment goals.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance.

• Educate patients and families about community resources available post-rehabilitation, ensuring they have the tools necessary for a successful transition back to daily life.

• Participate actively in discharge planning, facilitating smooth transitions for patients moving from our care to home or alternative care settings.
