This position is primarily onsite in the homes of patients in Woodbridge and the surrounding areas. Key Responsibilities

• Conduct thorough evaluations of patients' conditions and functional abilities through standardized assessments and observations in their home environments.
• Develop and implement individualized treatment plans that address patients' specific therapeutic goals, utilizing techniques such as therapeutic exercises, manual therapy, and modalities tailored to alleviate pain and enhance mobility.
• Regularly track and document treatments, patient responses, and progress to ensure accountability and adjust plans as necessary to optimize outcomes.
• Educate patients, families, and caregivers on the correct use of assistive devices and self-care techniques to enhance independent living.
• Participate in re-evaluations and modify treatment strategies in response to changes in patients' conditions or progress.
• Stay updated with the latest advancements in physical therapy practices through continuous professional development activities, including workshops, seminars, and collaboration with peers.

Patient Population / Client Focus

You will engage with diverse patient populations, including children and adults, who are navigating recovery from various conditions such as orthopedic injuries, neurological disorders, post-operative rehabilitation, or other physical ailments. Our home care approach allows the therapist to assess and treat patients in their natural environment, enhancing treatment effectiveness and patient comfort.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Valid state licensure to practice physical therapy.
• Exceptional communication skills, both written and verbal, to effectively convey treatment plans and instructions to patients and families.
• Proficiency in using electronic health records (EHR) and other necessary software for documentation and scheduling.
• Demonstrated ability to work independently and manage time effectively in a home care setting.

Preferred Experience

• A minimum of two (2) years of clinical experience in physical therapy, particularly in home care, is preferred.
• Knowledge of current best practices in physical therapy and rehabilitation.
• CPR certification and any additional certifications relevant to physical therapy practice.

Compensation and Benefits
