Work Environment

This role is primarily onsite as you will be providing physical therapy services directly in patients' homes. Key Responsibilities

• Conduct thorough physical therapy evaluations to assess patients' mobility, strength, balance, and functional capabilities.
• Design and implement individualized treatment plans utilizing therapeutic exercises, manual therapy techniques, and appropriate assistive devices to optimize physical performance.
• Deliver effective pain management strategies and educate patients on fall prevention measures tailored to their living environments.
• Track and document the progress of each patient, making necessary adjustments to therapy plans based on their evolving needs.
• Provide education to patients and their families on rehabilitation techniques and exercises to ensure continuity of care outside of therapy sessions.
• Collaborate with interdisciplinary teams, including nurses and physicians, to ensure seamless and integrated care delivery.
• Maintain meticulous clinical documentation in adherence to state regulations and company policies, ensuring accuracy and confidentiality.

Patient Population / Client Focus

Qualifications

• Current and valid Physical Therapy license in the state of Virginia.
• A Bachelor's degree in Physical Therapy; a Doctor of Physical Therapy (DPT) or Master's degree from an accredited program is preferred.

• A minimum of 2 years of experience in physical therapy, particularly in assisted living, skilled nursing, or geriatric settings.

Compensation and Benefits
