Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify patients' emotional, social, and financial needs.

• Provide individual and group counseling tailored to address adjustment disorders, grief, anxiety, and coping strategies related to terminal illness.

• Work collaboratively with interdisciplinary medical teams to develop and implement individualized care plans that enhance patient outcomes.

• Coordinate with community organizations to ensure post-discharge services are arranged seamlessly, fostering continuity of care.

• Offer emotional support and educational resources about treatment options, and help families navigate complex healthcare decisions.

• Engage actively in hospital committees, sharing expertise on issues related to social work and mental health services.

• Develop comprehensive discharge plans to ensure a smooth transition to community resources for ongoing support.

• Maintain accurate documentation in compliance with hospital policies and relevant legal standards.

• Provide crisis intervention and immediate support to patients and families during moments of acute distress.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
