Key Responsibilities\n

• Conduct thorough bio-psychosocial evaluations to assess and diagnose a wide range of psychosocial issues.

• Develop and implement individualized treatment plans that incorporate evidence-based modalities aligned with each patient's needs.

• Provide both individual and group therapy sessions, consistently analyzing treatment efficacy and adapting approaches as necessary to maximize patient outcomes.

• Collaborate with a multidisciplinary team, offering psychological insights and advocacy for patients' unique circumstances to optimize care coordination.

• Document all social work activities comprehensively in patients' medical records, adhering to strict departmental policies regarding documentation and compliance.

• Educate patients and families about medical and therapeutic recommendations to facilitate understanding and compliance, aiding in the restoration of optimal functioning.

• Stay informed about community resources and offer guidance to patients regarding advocacy and support options available to them.

Patient Population / Client Focus\n

The LCSW will primarily work with patients facing complex psychosocial challenges as a result of chronic or terminal illnesses. This role requires a deep understanding of the emotional and social factors that affect patient experiences and outcomes, with a strong emphasis on providing culturally sensitive care that respects each individual's unique background.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, along with a current LCSW license from the State Board of Social Work Licensure.
