This role is located onsite at our fully equipped rehabilitation facility in Keyport, NJ. Key Responsibilities

• Conduct thorough evaluations of patients by reviewing medical history and performing specific assessments to determine individualized treatment plans.
• Communicate evaluation findings and develop treatment strategies in collaboration with patients and their families, ensuring clarity and understanding throughout the rehabilitation process.
• Implement evidence-based treatment protocols tailored to the unique needs and evolving conditions of patients, while keeping abreast of advancements in physical therapy methodologies.
• Coordinate with other healthcare professionals to customize intervention strategies, and promptly recognize and document any changes in patient conditions that may necessitate modifications to treatment plans.
• Ensure meticulous and timely documentation of patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations, capturing the entire journey of the patient's rehabilitation.
• Demonstrate effective time management to meet productivity goals while maintaining a flexible schedule. Participate in marketing initiatives to promote community awareness of our services.
• Contribute to department goals by participating in quality improvement projects and compliance with accreditation standards, alongside a commitment to continuing education and professional growth.
• Engage in clinical instruction or mentorship roles for students and junior professionals, promoting knowledge transfer within the team.
• Encourage excellent customer experiences and adaptability in responding to patient needs, ensuring seamless access to care.

Patient Population / Client Focus

At Aviant, you will work with a diverse range of patients, including individuals recovering from injuries, surgeries, and chronic conditions. Your role will involve developing therapeutic interventions aimed at restoring mobility, reducing pain, and helping patients regain independence. Qualifications

• Bachelor's Degree in Physical Therapy or a related field.
• A valid DPT (Doctorate in Physical Therapy) license in the State of New Jersey is required.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

• At least 2 years of professional experience as a Physical Therapist is preferred, although recent graduates are encouraged to apply.
