Key Responsibilities\n

• Act as a vital liaison between clients, their families, and external outpatient providers to ensure a continuous support system.

• Collaborate with insurance companies for pre-certification and utilization management, facilitating appropriate care plans for clients.

• Participate actively in designing and executing discharge and aftercare plans that align with individual client needs and recovery goals.

• Provide therapeutic meal support to clients, encouraging the development of healthy relationships with food through modeling and guidance.

Qualifications\n

Compensation and Benefits\n
