Key Responsibilities\n

• Act as a crucial liaison with families and outpatient providers, ensuring a coordinated approach to care that meets the unique needs of each client.

• Engage with insurance providers for the purposes of pre-certification and utilization management, advocating for clients to secure the necessary resources for their recovery journey.
\n
• Participate in comprehensive discharge and aftercare planning, creating personalized transition plans that facilitate ongoing support and community integration.

• Provide therapeutic meal support, actively working to model a healthy relationship with food, and educate clients about its connection to emotional well-being.
\n
• Deliver empathetic, safe, and affirming care, respecting clients' intersecting identities and individual circumstances, to promote a sense of belonging and understanding.

Patient Population / Client Focus\n

This role involves working primarily with clients experiencing substance use disorders, many of whom may also face mental health challenges. You will engage with a diverse population, necessitating cultural competence and an understanding of various lived experiences. Tailoring your approach to each client’s specific challenges will be key in their recovery success.

Qualifications\n

• Education: Master’s degree in Social Work from an accredited institution.

• Licensure: A valid LCSW license in the state of Tennessee is required.

• Experience: Proven experience working with older adults in a therapeutic setting.

• Skills: Strong clinical and diagnostic skills, coupled with excellent communication and interpersonal abilities.

Compensation and Benefits\n
