Description

• Assess patients in various settings, both inpatient and outpatient, and create tailored treatment plans
• Implement therapeutic interventions aimed at improving mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and rehabilitation teams to ensure optimal patient outcomes
• Educate patients and their families about therapeutic objectives, discharge planning, and exercises for home care
• Keep detailed documentation of evaluations, treatment sessions, and patient progress in accordance with clinical guidelines
• Engage in care team rounds and multi-disciplinary meetings to foster cooperation and continuity of care

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• Certification in CPR preferred
• Additional specialized certifications (e.g., neurological or geriatric) are advantageous but not necessary
