Description

• Assess patients in inpatient and outpatient contexts and craft tailored treatment regimens
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate cohesively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families in therapeutic objectives, discharge planning, and exercises for home
• Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage actively in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not required

Skills

• Previous clinical experience is highly desirable
• Ability to manage patients with complicated health and mobility challenges
• Excellent communication and teamwork skills
