Key Responsibilities\n

As a Licensed Clinical Social Worker with Transformative Healing Services, your primary responsibilities will include:

• Conducting comprehensive clinical assessments of clients to formulate individualized treatment plans based on established best practices and evidence-based interventions.

• Delivering various therapeutic modalities, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Solution-Focused Brief Therapy (SFBT) tailored to meet the unique needs of each client.

• Maintaining accurate, timely, and secure clinical records in compliance with HIPAA and other relevant regulations to ensure patient confidentiality and quality of care.

• Collaborating actively with a multidisciplinary team, composed of psychiatrists, psychologists, and other social workers, to ensure a holistic and effective approach to patient treatment.

• Participating in regular case consultations and peer review meetings that foster learning, improve care strategies, and promote high standards within the team.

• Educating patients and their families on mental health issues, treatment options, and community resources, equipping them with tools to manage their health journeys effectively.

Patient Population / Client Focus\n

Your work will involve serving a diverse patient population that includes individuals from various backgrounds and with a wide range of mental health issues including anxiety, depression, trauma, and grief, particularly within a hospice setting. You will help support families coping with life-limiting illnesses, ensuring they receive the care needed to navigate these challenges while enhancing their emotional resilience and overall well-being.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in New York.

Compensation and Benefits\n
