Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to identify developmental needs and challenges.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP), collaborating closely with families and interdisciplinary teams.

• Monitor and track outcomes related to the IFSP, ensuring families receive the appropriate support and adjustments are made as necessary.

• Provide guidance and support to families based on identified needs, utilizing evidence-based practices to enhance the child's developmental trajectory.

• Utilize the coaching model and Natural Learning Environment Practices to create effective learning experiences for families and children.
\n
• Foster trusting relationships with children and their families to promote engagement and successful interventions.
\n
• Ensure compliance with all federal and state regulations governing social work services.
\n
• Maintain clear, accurate, and timely documentation of assessments, treatment plans, and family interactions.

Patient Population / Client Focus\n

This role primarily serves infants and toddlers experiencing disabilities or developmental delays. As an LCSW, you will work directly with families to navigate challenges while promoting positive developmental outcomes in physical, cognitive, social/emotional, and communication domains. Your role is pivotal in helping families understand and utilize available resources, empowering them through active support and information sharing.
