Work Environment

As a Physical Therapist at Headstrong Rehabilitation & Wellness, you will work onsite primarily in the home care setting, which allows for personalized interaction with patients in their own environments. Key Responsibilities

• Conduct comprehensive evaluations of patients, gathering detailed medical histories and performing specific assessments to design tailored treatment plans.
• Develop and implement effective treatment protocols based on patient evaluations, ongoing assessments, and the latest evidence in rehabilitation.
• Collaborate actively with other healthcare professionals to ensure coordinated care that may involve adjustments in the treatment plan as patient conditions evolve.
• Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress notes to ensure compliance and continuity in care.
• Participate in discharge planning, detailing follow-up care to optimize recovery and prevent rehospitalization.
• Provide ongoing education to patients and their families about rehabilitation exercises and strategies that promote ongoing health and wellness.
• Attend and contribute to facility meetings, sharing insights and engaging in discussions about clinical practice to improve patient outcomes.

Patient Population / Client Focus

This role involves working primarily with orthopedic patients who require rehabilitation services Post-surgery, injury recovery, or chronic pain management. You'll be directly impacting the lives of diverse individuals, empowering them to regain strength, mobility, and independence in their daily activities.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current and valid physical therapy license to practice in New Jersey.
• American Heart Association's BLS for Healthcare Providers certification.

Preferred Experience

We are looking for candidates with a minimum of two years of experience in clinical physical therapy settings, particularly those who have worked in home care or outpatient services. Familiarity with orthopedic rehabilitation is strongly preferred, as is a proactive approach to patient-centered care.

Compensation and Benefits
