Work Environment

This position will be conducted primarily onsite at our partnered assisted living facilities in the region. Key Responsibilities

• Deliver individualized onsite therapy services to residents in various assisted living communities, utilizing evidence-based practices to drive rehabilitation progress.
• Conduct thorough evaluations of new patients, developing personalized orthopedic-informed treatment plans designed to meet each resident's unique challenges and goals.
• Collaborate effectively with facility nursing staff, caregivers, and our internal multidisciplinary team to coordinate care and enhance patient outcomes.
• Provide education to residents and families regarding safe mobility practices, home exercise programs, and overall wellness strategies, empowering them to take charge of their health.
• Utilize mobile EMR systems to document therapy sessions and patient progress, ensuring all records maintain compliance with healthcare regulations.
• Serve as a liaison between assisted living facilities and our clinic, facilitating referrals and follow-up appointments to support ongoing patient care.

Patient Population / Client Focus

You will primarily work with older adults residing in assisted living facilities, many of whom may experience orthopedic issues, mobility challenges, or require general physical rehabilitation services. Your role is critical in fostering their independence and enhancing their quality of life through tailored therapeutic interventions.

Qualifications

• Possess a Bachelor's Degree in Physical Therapy from an accredited institution.
• Hold an active Physical Therapist license in the State of New Jersey or be eligible to obtain one.
• Demonstrate excellent communication and interpersonal skills, enabling effective interactions with residents, family members, and the healthcare team.
• Exhibit solid clinical knowledge concerning orthopedic conditions prevalent among the elderly population.
• Be comfortable working independently while managing your schedule and case load in a residential setting.

Preferred Experience

• Minimum of two years of experience in outpatient, geriatric, or assisted living settings, especially in roles focusing on rehabilitation.
• Experience with electronic medical records systems, enhancing your efficiency in providing patient care and documentation.
