Work Environment

This full-time role requires the Physical Therapist to work onsite within our newly developed assisted living facilities. Here, you will collaborate closely with nursing staff, occupational therapists, and other healthcare providers to deliver a continuum of care that enhances patient outcomes. Your expertise will be utilized in developing and implementing rehabilitation programs that are conducive to a supportive and nurturing environment, aiming to restore independence and improve functional capabilities in our elderly clientele.

Key Responsibilities

• Conduct thorough evaluations to identify patients' individual goals and needs, ensuring a tailored approach to each treatment plan.
• Implement a range of interventions aimed at improving motion and mobility, utilizing evidence-based practices that minimize reliance on medications and avoid unnecessary surgeries.
• Design and develop comprehensive care plans that incorporate diverse treatment techniques to meet the needs of geriatric patients.
• Lead the creation of wellness programs focusing on fitness tailored to strengthen, enhance flexibility, balance, and coordination among patients.
• Motivate and engage patients throughout their rehabilitation journey, establishing therapeutic relationships that promote trust and cooperation.
• Monitor and evaluate patient progress, making adjustments to care plans as necessary and maintaining accurate documentation for all services rendered.
• Collaborate with other healthcare professionals to create an integrated approach to patient care, emphasizing effective communication for optimal outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state license to practice Physical Therapy is required.
• Demonstrated experience in a similar setting, particularly with geriatric patients, is preferred.

While a bachelor's degree in Physical Therapy is essential, candidates with experience in geriatrics, orthopedics, and physical rehabilitation medicine will be at an advantage. Compensation and Benefits
