Developmental Center Placement Worker - Moreno Valley Adult/State Developmental Centers

SUMMARY: Under the direct supervision of the Program Manager – State Developmental Centers (SDC), is responsible for the direct case management services offered to consumers by the program; serve consumers and sees to it that entire case management process with concomitant authorizations and paperwork is completed; has advocacy responsibilities and may be assigned liaison tasks. A high priority is placed on moving consumers from the SDCs to the community. Perform related work as assigned. Has no supervisory responsibility.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Guided by the agency’s and program’s mission statement and core values, provides services to consumers in SDC’s and perform liaison activities as required. Complete all necessary paperwork, reports, etc. on a timely basis.
• Daily, complete administrative requirements including, but not limited to, those related to Eligibility Review, Special Incident Report (SIR) closures, Quality Alerts, and Consumer File Record Documentation in accord with Agency's Policies and Procedures. Completion of at least 95% required case-related paperwork within designated time frames. Provide written reports of activities to the Program Manager – SDC quarterly. Complete appropriate Targeted Case Management (TCM) documentation accurately and on a timely basis.
• Coordinate, implement and update the Annual Review and Individual Program Plan (IPP) process on an annual basis. Maintain contact with consumers and their families on a quarterly basis. Provide information and referral services. Provide advocacy services to consumer and his family. Seek out and effectively utilize generic resources on behalf of consumer. Facilitate services identified in IPP.
• Evaluate consumer progress and plan as identified in IPP on an ongoing basis to include need for community placement. Review consumer assessment including medical, psychological/developmental, educational or vocational and others as needed. Is well prepared for Compliance Review, eligibility review and other administrative case reviews.
• Maintain MediCaid Waiver standards.
• Maintain a working knowledge of the Lanterman Act that pertains specifically to the Developmental Center population and those individuals involved in the legal system. Act as liaison with designated courts. Is responsible for completing and submitting court reports on a timely basis.
• Arrange for placement of consumers from SDC into community placement facilities when appropriate. Upon placement also arrange for day program, transportation and any other service identified in IPP prior to transferring case to appropriate community program. Provide information for the development of the agency’s Community Placement Plan (CPP).
• Complete individual or family crisis intervention and appropriately document activities. Complete SIR on a timely basis and inform representatives of other agencies and programs as appropriate.
• Identify situations in vendored programs requiring the attention of Quality Assurance (QA) and make appropriate referral. Completes a facility audit protocol (ICRC 513) and forward to the Quality Assurance program when facility problems are identified. As assigned do an unannounced facility audit, completing the ICRC 513 and forwarding it to the Quality Assurance program. Lead or participate in at least one Quality Assurance evaluation/audit of a community care facility, a health care facility, or a day program annually.
• Perform SDC liaison duties daily, as assigned. Act as the program’s/Regional Center’s liaison to at least two community agencies, parent groups or programs; attend meetings, provide information about the program/Regional Center and act as a resource for that program. Represent the regional center at case conferences and interagency meetings.
• Keep Program Manager - SDC Program informed of the status of the caseload; consumer needs and progress; SDC liaison activities, and unusual or difficult case situations.
• Assist in sponsoring new employees as assigned.
• Daily, maintain an adequate skill level in interpersonal and community relationships. Perform as a member of a team, answering questions, sharing expertise and generally contributing to the harmony of the team. Upon request provide complete case management coverage to consumers whose Consumer Services Coordinator is absent.
• Comply with Agency's Injury and Illness Prevention Plan. Notify Program Manager – SDC of any unsafe working conditions. Use equipment accurately and keep it in good repair. Keep work area neat and orderly and free from hazards. Observe all safety rules.
• Successfully complete all assignments arising out of agency’s Performance Contract as applicable.
• Organize travel efficiently and effectively. Maintain a safe driving record. In the event part of the record is out-of-date, make arrangements to correct the deficiency.
• Utilize agency’s Information Technology systems as assigned, maintaining security and following agency’s protocol and procedures, rules and requirements.
• Handle change well. Is flexible and adaptable in dealing with interruptions, new priorities and new assignments. Aggressively seek to maintain currency and expand relevant knowledge base. Attend training sessions as required.
• Comply with the Agency's Affirmative Action Plan. Comply with and ensure compliance with the Agency's Personnel Policies and Procedures. Assure consumers' rights and dignity are maintained in the provision of services.
• Maintain good attendance and punctuality.
• Perform different or additional work as assigned.

MINIMUM POSITION REQUIREMENTS:

• Master’s Degree in social work, psychology, public health nursing, or a related field from an accredited school.
• Or a BA degree in social work, psychology, or a related field plus one year of experience in developmental disabilities or a related field. Or a BS in nursing from an accredited school, plus one year of experience in developmental disabilities or a related field.
• Full use of an automobile, possession of a valid California driver's license and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation. Must have and maintain a safe driving record.
• Bilingual preferred.
