Key Responsibilities\n

• Conduct therapeutic sessions via telehealth platforms, providing psychotherapy and tailored mental health services to patients at the hospice center.

• Utilize a variety of psychological assessments and diagnostic tools to evaluate patients’ mental health and to inform treatment planning.

• Create individualized treatment plans based on rigorous assessments and continuous monitoring of patient progress, adjusting therapeutic approaches as needed.

• Implement crisis intervention strategies as necessary, including conducting suicide assessments and facilitating hospitalizations when required.

• Collaborate proactively with a multidisciplinary team, including physicians, nurses, and other health professionals, to create a comprehensive support system for our clients.

• Maintain precise and timely records of patient interactions and therapeutic services in compliance with organizational policies and HIPAA regulations.

• Contribute to the development of clinical policies and help shape standardized procedures that enhance service delivery.

• Engage actively in staff meetings and continuing education opportunities to remain current with best practices in mental health care.

In this role, you will primarily serve individuals and families coping with terminal illnesses in a hospice setting. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, coupled with a current and valid LCSW license.
