Key Responsibilities\n

As a Licensed Professional Counselor, your primary responsibilities will include:

• Conducting comprehensive assessments to determine individual patient needs and goals.

• Maintaining consistent communication with supervisory staff, family members, and healthcare team members regarding patients' progress.

• Developing and sharing a tentative schedule of appointments and home visits to ensure consistent follow-ups and monitoring.

• Coordinating essential home visits that facilitate direct observations, interviews, and data collection, while adhering to privacy and ethical guidelines.

• Ensuring that program plans are effectively implemented and periodically evaluated for patient efficacy.
\n
• Participating in interdisciplinary team meetings to discuss case progress and share best practices with fellow counselors.

• Completing monthly and quarterly progress notes, ensuring that pertinent data is accurately recorded and communicated.

• Organizing and leading training sessions for staff and team members on the implementation of care programs and documentation protocols.

• Conducting follow-up interviews and home visits, providing ongoing support and adjustments to care plans as necessary.

• Maintaining detailed and organized records to facilitate information sharing with relevant stakeholders.
