Key Responsibilities\n

• Conduct thorough reviews of patient health histories in preparation for initial evaluations and ongoing assessments.

• Develop, implement, and regularly reevaluate individualized mental health care plans that align with established clinical standards and best practices.

• Engage with patients and their families to identify educational needs, establish treatment goals, and craft comprehensive plans of care.

• Evaluate patient responses to educational interventions, modifying care plans as necessary to optimize patient outcomes.

• Collaborate closely with physicians and other interdisciplinary team members to coordinate comprehensive care strategies and effectively communicate about patient status.

• Facilitate appropriate referrals to other healthcare professionals when integration of additional support is indicated.

• Uphold patient confidentiality in compliance with HIPAA regulations while managing medical records and documenting care activities accurately.

• Lead and conduct engaging healthy minds workshops aimed at educating patients and families on mental health fundamentals and coping strategies.

Qualifications\n

Compensation and Benefits\n
