Job Summary: Home Health Nurse (HH Hospice) Bristol, Connecticut
The organization is seeking a Home Health Nurse to provide home health and hospice nursing care for adults and geriatrics. Responsibilities include evaluations, treatment, and development of a plan of care, with a focus on common home-care and hospice diagnoses. Candidates are required to bring their license on the first day of employment.

Job Details:

• Schedule: Days, 8 hours per shift, minimum 40 hours per week; MondayFriday 8:00 AM4:30 PM with weekend call rotation (flexible day coverage from the MF schedule as needed)
• Required Qualifications: Basic Life Support (BLS) certification; valid drivers license; current auto insurance; must be licensed and fingerprinted in Connecticut; 1 year of home care or hospice experience
• Preferred Qualifications: Experience with Oasis (Home Health) and Meditech home care Electronic Medical Record (EMR)
• Responsibilities: Perform evaluations and treatment; develop and follow the plan of care; support medication management and care needs related to common diagnoses including post-surgical orthopedic (total joints), cerebrovascular accident (CVA), dementia, hospice care, and conditions such as chronic obstructive pulmonary disease (COPD)
• Additional Information: Weekend call coverage is required, with callback to the client within 20 minutes and a visit within 30 minutes on weekends; charting is completed in Oasis (Home Health); home health/hospice aides are available
