Work Environment\n

The selected candidate will work remotely, engaging with clients virtually and occasionally in person when necessary. Our full-time schedule runs from Monday to Friday, with typical hours from 8:00 AM to 4:30 PM. The flexibility in scheduling is essential to accommodate client needs, and candidates should be prepared for occasional evenings or weekends, as well as being on-call for urgent situations. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand the emotional, mental, and social factors influencing clients' well-being.

• Develop and implement tailored care plans in collaboration with clients and their families to meet their specific goals and preferences.

• Provide counseling and emotional support to seniors, assisting them through life transitions and challenges.

• Advocate for clients' rights, helping them access community resources and support services to enhance their quality of life.

• Collaborate with a multi-disciplinary team of healthcare professionals, including nurses and caregivers, to ensure coordinated and comprehensive care.

• Regularly visit clients to monitor their well-being, evaluate living conditions, and address any concerns related to their health and safety.

• Facilitate constructive communication among family members to resolve conflicts related to the care of the senior client.

• Educate clients and families about available community resources, support groups, and programs that promote social interaction and engagement.

• Maintain meticulous documentation of client assessments, care plans, progress notes, and all relevant interactions to ensure compliance with best practices.

Qualifications\n

Compensation and Benefits\n
