This remote position allows for flexible work arrangements while maintaining a strong connection to our outpatient office. Key Responsibilities\n

• Conduct comprehensive assessments and develop detailed treatment plans for seniors, including pre-crisis and crisis intervention plans, behavioral guidelines, and medication treatment strategies.

• Ensure accurate and timely completion of all required documentation that meets regulatory compliance.

• Utilize evidence-based screening tools and methods in collaboration with the clinical team to promote effective care outcomes.

• Work alongside interdisciplinary teams to formulate and execute positive behavior support plans tailored to individual client needs.

• Play an active role in all licensing and accreditation processes, contributing to the overall operational excellence of the clinic.

• Act as a liaison with relevant service systems, state agencies, and community partners to coordinate care and resources for our clients.

Patient Population / Client Focus\n

Compensation and Benefits\n
