Able Home Healthcare is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in San Diego, CA. Responsibilities:

Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans

Provide skilled PT interventions including therapeutic exercise, gait training, and neuromuscular re-education

Monitor and document patient progress, adjusting care plans as clinically appropriate

Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care

Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques

Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards

Manage a caseload of home health patients across the San Diego area with a flexible schedule

Requirements:

Active California Physical Therapist (PT) license in good standing

Graduate of an accredited Physical Therapy program (DPT or MPT)

Home health or outpatient orthopedic/neurological experience preferred

Strong knowledge of Medicare home health guidelines and documentation requirements

Excellent communication, time management, and organizational skills

Valid California driver's license and reliable personal vehicle with current auto insurance

Compassionate, self-motivated, and comfortable working independently in a field-based setting
