Key Responsibilities\n

• Conduct thorough psychosocial assessments via interviews, observations, and analysis of medical records to gain insight into the emotional, social, and environmental challenges impacting patient recovery.

• Provide empathetic counseling and support services to patients and their families, helping them address emotional adjustments and well-being throughout the rehabilitation journey.

• Collaborate closely with the rehabilitation team to create and implement personalized treatment plans that prioritize each patient's psychosocial needs and improve their overall support systems.

• Assist patients in navigating and accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance, to foster independence post-rehabilitation.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A valid state license as a Licensed Clinical Social Worker (LCSW).
