Key Responsibilities

• Conduct thorough Physical Therapy evaluations for children and adults, gathering crucial data to inform treatment plans.
• Design, implement, and regularly update individualized treatment plans that align with each patient's specific rehabilitation goals.
• Reassess clinical signs and symptoms periodically to evaluate the effectiveness of interventions and make necessary adjustments to treatment strategies.
• Maintain accurate and timely documentation, including SOAP notes and progress summaries, utilizing our electronic health record system.
• Educate patients and their families about the recovery process, setting realistic expectations and addressing potential challenges during therapy.
• Achieve minimum utilization targets and contribute to broader business objectives through quality care delivery.
• Collaborate effectively with multidisciplinary teams to ensure comprehensive patient care when clients are receiving services from multiple providers.
• Promote and embody the core values of Heal Motion Care, acting as a brand ambassador in every interaction with patients and colleagues.

Patient Population / Client Focus

The Physical Therapist will work primarily with a diverse patient population that includes children and adults facing various health challenges. This role demands a flexible approach to treatment, as you will engage with clients from different backgrounds and cultures, tailoring interventions to meet their unique needs and preferences.

Qualifications

• A Master's degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program is required.
• Current, unrestricted licensure from the Board of Physical Therapy to practice in the state of California.
• Must possess the ability to conduct evaluations and provide treatments in-person and via telehealth, when clinically appropriate.
• A strong desire to work with a diverse array of patients across various age groups, ethnicities, and backgrounds.
• The ability to adapt quickly to changing circumstances, demonstrating flexibility in approach and treatment modalities.
• Strong collaboration skills and a commitment to delivering exceptional patient-centered care.

Preferred Experience

Experience in treating pediatric and geriatric populations, along with familiarity with state-of-the-art rehabilitation techniques and technologies, is highly preferred. Participation in continuing education and professional development is encouraged as part of your growth within the organization.

Compensation and Benefits
