Work Environment\n

This role is entirely remote, which allows for flexibility in your work schedule while ensuring that you can work from anywhere in the Bakersfield, California area. As part of a community health clinic, you will be equipped with the necessary tools to perform your job effectively, including a comprehensive electronic medical records (EMR) system that streamlines patient management and documentation processes. You'll collaborate with a multi-disciplinary team, involving various stakeholders to create optimal care plans.

Key Responsibilities\n

• Conduct thorough assessments to evaluate the needs of each patient, developing personalized treatment plans that cater to their unique circumstances.

• Maintain effective communication with supervisors, support staff, and family members regarding ongoing patient progress, ensuring that everyone involved is informed and engaged in the treatment process.

• Schedule and coordinate appointments and home visits, providing your supervisors with a tentative calendar for patient interactions and evaluations.
\n
• Lead in-home observations, chart reviews, and data analysis to gain insight into client behaviors and progress, actively participating in the formulation and adjustment of treatment strategies based on findings.

• Oversee the implementation of program plans, ensuring that they are executed correctly, while also providing ongoing training to team members for competence in these tasks.

• Document monthly and quarterly progress notes to consistently track patient outcomes and improve care delivery.
\n
• Organize training sessions for staff to promote a high standard of service delivery and ensure compliance with documentation standards.
\n
• Actively participate in Interdisciplinary Team Meetings to collaborate on patient care and share best practices within the team.
\n
• Serve as a liaison with the public and mental health community, driving initiatives that support developmental goals and access to resources.

• Engage in follow-up interviews and observations, ensuring continued support for individuals as they progress through their treatment plans.

Patient Population / Client Focus\n

In this role, you will have the opportunity to work with diverse populations, including both adults and youth dealing with various mental health challenges. This includes, but is not limited to, family therapy, trauma recovery, substance misuse, and general counseling. Qualifications\n

• Must possess a Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted license as a Professional Clinical Counselor (LPCC) in California is required.

• A minimum of 2 years of clinical counseling experience is mandatory, with a strong preference for experience in an outpatient mental health setting.

• Specialization or training in areas such as family therapy, couple therapy, trauma-informed care, or substance misuse is highly desirable.

• Demonstrable understanding of legal and ethical standards in mental health practice.

• Proficient interpersonal and communication skills, particularly in establishing rapport with clients from various backgrounds.

• Proven ability to work collaboratively within a multidisciplinary healthcare team.

• Competency with electronic medical records (EMR) software and other relevant technologies.

Preferred Experience\n

While the essential qualifications include a solid foundation in mental health counseling, candidates with experience in specialized therapeutic practices or community-based care will stand out.
