Job Summary Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role you’ll primarily travel — up to 100% of the time — across a defined coverage area. This may require overnight hotel stays, car rentals and/or flights, depending on the prescribed coverage area. Holding multiple state licenses will be required, as dictated by business needs. Responsibilities Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Conduct in‑home visits that last about 45 minutes, connecting one‑on‑one and answering health questions, including medication and medical history review, physical evaluation, and optional diagnostic tests. Use an iPad and an intuitive clinical workflow for each evaluation and perform services including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams). Receive additional per‑completion incentive pay for all in‑home visits. Be flexible to travel locally and within licensed states, up to 100% of the time. Conduct virtual visits as needed. A Note on Our Work Environment As an in‑home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions. Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Board certification (required). Able to obtain and maintain multi‑state licenses in coverage area, as required. Able to cover all counties within New Hampshire and flex to other areas of business need. NH Coverage Market Areas include the following counties: Rockingham, Strafford, Belknap, Merrimack, Sullivan, Coos, Carroll, Grafton, Cheshire, Manchester, and Hillsborough. Our concierge‑style Network Activation team will support you in obtaining, renewing, enrolling or verifying additional state licensure in new states as business needs exist. Compensation & Work Hours Anticipated weekly hours: 40. Pay range: $87,035.00 – $187,460.00. This represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls.
