Key Responsibilities\n

• Conduct thorough assessments of clients' needs by either phone or in-person, gathering relevant social history and emotional state.

• Develop individualized support plans that are comprehensive and measurable, tailored to the specific issues that each client faces.

• Implement intervention strategies, providing therapeutic counseling and crisis intervention as necessary during client sessions.

• Monitor client progress, regularly evaluating and adjusting treatment plans to ensure they remain effective and aligned with client goals.

• Maintain meticulous documentation of client interactions and treatment processes in accordance with organizational policies and state regulations.

• Participate actively in clinical supervision and case conferences, offering insights and collaborating with other social workers, therapists, and healthcare providers.

• Serve as a key worker, linking clients effectively to community resources such as support groups, mental health services, or substance abuse treatment programs.

Qualifications\n

Compensation and Benefits\n
