Work Environment

The position is entirely onsite in home care settings , allowing you to meet patients where they are most comfortable. You will travel within Fresno and the surrounding communities, providing flexible and personalized therapy sessions. Key Responsibilities

• Perform comprehensive evaluations of patients, interpreting examination findings to determine treatment needs, including the nature and extent of dysfunction related to various medical and health-related conditions.
• Develop and implement individualized treatment plans based on patient assessments, aiming to restore maximum functionality and improve quality of life.
• Monitor and document patient progress meticulously, ensuring adherence to regulatory, licensing, and payer requirements, utilizing industry-standard EMR systems.
• Collaborate with a multidisciplinary team, including physicians, nurses, and rehabilitation specialists, to communicate patient status and treatment outcomes effectively.
• Instruct patients' families and caregivers in follow-through programs to promote long-term health and wellness beyond formal therapy sessions.
• Maintain the cleanliness and safety of all equipment and work areas, complying with health and safety standards.
• Uphold a high standard of professional conduct in accordance with our company's Code of Business Conduct and applicable laws.

Patient Population / Client Focus

You will work with a diverse population of patients, ranging from post-operative individuals to those with chronic pain and mobility issues, including pediatric and geriatric clients. Your ability to tailor treatment plans to accommodate different age groups, medical histories, and personal goals is essential for their recovery.

Qualifications

• Current and valid physical therapy licensure in the State of California.
• National Provider Identifier number.
• A Bachelor's Degree in Physical Therapy from an accredited program.
• Strong foundation in orthopedic and sports rehabilitation practices.

Preferred Experience

• A minimum of two years of experience in a clinical setting, with a focus on orthopedic and musculoskeletal rehabilitation.
• Proven knowledge of evidence-based practices in physical therapy, with the ability to design appropriate exercise and rehabilitation programs.
• Excellent communication skills with a strong emphasis on patient education and engagement.

Compensation and Benefits
